Global longitudinal strain: a useful everyday measurement?

چکیده
Herceptin (Trastuzumab) is a widely used and effective drug for the treatment of Her2+ breast cancer but its cardiotoxic side effects require regular monitoring by echocardiography. A 10% reduction in left ventricular ejection fraction can lead to suspension of treatment and therefore has significant implications for patient prognosis in terms of cardiac and cancer outcomes. Assessment of LV function by conventional 2D biplane method of discs (2DEF) has limitations in accuracy and reproducibility. Global longitudinal strain (GLS) is becoming more widely available and user friendly. It has been shown to demonstrate myocardial damage earlier in treatment than 2DEF, allowing the option of pharmacological intervention at a pre-clinical stage and preventing the interruption of Herceptin. This study compares the reproducibility of GLS with that of 2DEF in a routine clinical environment. Fifty echocardiograms performed on female patients undergoing Herceptin treatment were used to measure both 2DEF and GLS within the recommended standard appointment time of 40 min. The data were re-measured (blind) by the same operator a minimum of 14 days later to determine intra-operator variation. These data were also measured by a second operator (blind), to assess inter-operator variation. Analysis by direct comparison, intra-class correlation (ICC), coefficient of variation (CV) and Bland-Altman plots demonstrated that GLS is a more reproducible measurement than 2DEF. This is important to prevent clinical decisions being erroneously based on variation in operator measurement. The investigation also shows that with advances in machine software this is a practical addition to routine assessment rather than merely a research tool.

Evaluation of Global Longitudinal Strain Pattern (GLS) and Echocardiographic Parameters in the Sleeve Gastrectomy Patients Before and After Surgery
Background and Objective: Obesity is one of the problems associated with cardiovascular complications. Patients' heart function improves after sleeve gastrectomy. Global Longitudinal Strain (GLS) is one of the methods used to monitor heart function before and after sleeve gastrectomy. In this study, we evaluated the echocardiographic parameters pre and post sleeve gastrectomy. Materials and M...

The study aimed was to measure left ventricular (LV) strain using speckle tracking echocardiography (STE) for assessment of LV function. Eighty-two subjects (mean age 57±9 years) with suspected chest pain underwent two-dimensional (2D)-echocardiography before coronary angiography. Conventional echocardiographic parameters were used for the assessment of LV function. Longitudinal strain and its ...
متن کاملAssessment of right ventricular global and regional longitudinal peak systolic strain, strain rate and velocity in healthy fetuses and impact of gestational age using a novel speckle/feature-tracking based algorithm.
OBJECTIVE To evaluate the correlation between feature tracking-derived measurements of the right ventricular myocardium and gestational age in healthy fetuses. METHODS Global and segmental longitudinal peak systolic strain, strain rate and velocity values of the right ventricular myocardium were assessed by a novel feature-tracking technique in 150 healthy fetuses at between 13 and 39 weeks' ...
متن کاملMechanical behavior measurement of the sheep small intestine using experimental tests
Background: There is no consistent data on the mechanical properties of sheep intestine. OBJECTIVES: We performed a series of biaxial strain measurement experiments and extracted the constitutive model to describe the mechanical characteristics of the sheep intestinal tissue. METHODS: Eleven specimens were obtained freshly from sacrificed sheep and the planar biaxial tests were performed on th...
متن کاملThe measurement of everyday cognition (ECog): scale development and psychometric properties.
This article describes the development and validation of an instrument to assess cognitively mediated functional abilities in older adults, Everyday Cognition (ECog). The ECog is an informant-rated questionnaire comprised of multiple subscales. Confirmatory factor analysis (CFA) was used to examine its factor structure. Convergent validity was evaluated by comparing it to established measures o...
